CKD patients typically develop gut dysbiosis characterized by overgrowth of urease- and indole-producing bacteria (e.g., Clostridiaceae, Lachnospiraceae) and depletion of beneficial short-chain fatty acid (SCFA) producers.

20-30% reduction in plasma homocysteine with B6+B9+B12 triad in B-vitamin-replete CKD patients — combined with SCFA microbiome shift reducing uremic toxin production at the source.
Why Nephro Forte works.
Nephroforte is a clinically advanced nephrological synbiotic designed for Chronic Kidney Disease (CKD) management via enteric dialysis - leveraging the gut microbiome to reduce protein-bound uremic toxins that standard hemodialysis cannot effectively clear.

How it works, step by step.
The documented biological pathway — exactly how each component acts inside the body.
This imbalance increases production of harmful protein-bound uremic toxins - indoxyl sulfate (IS), p-cresyl sulfate (pCS), and TMAO -which drive renal fibrosis, endothelial dysfunction, and 10- to 20-fold higher cardiovascular mortality.
Nephroforte counters this by:
Shifting the microbiome from pro-uremic to protective, SCFA-dominant composition, thereby reducing toxin generation at the source.
Correcting methylation pathway deficiencies with the B-vitamin triad to lower elevated homocysteine (an independent CV risk factor in CKD).
B9 (folate) + B12: Support remethylation of homocysteine.
B6 (pyridoxine): Enhances the transsulfuration pathway, converting homocysteine to cysteine.
Bacillus coagulans and Bifidobacterium longum competitively displace urease-producing bacteria in the colon via bacteriocin production and short-chain fatty acid (SCFA)-induced pH lowering.
This directly inhibits urea hydrolysis to ammonia - the main driver of enteric urea nitrogen recycling and azotaemia in CKD.
The postbiotic fraction (heat-inactivated cell wall components including peptidoglycans, lipoteichoic acids, and exopolysaccharides) activates pattern recognition receptors (PRRs), lowering systemic LPS load and CKD-associated inflammation independently of live bacteria.
B vitamins further support this: B9 (folate) and B12 enhance methionine synthase activity for homocysteine remethylation, while B6 (as pyridoxal-5-phosphate) activates cystathionine beta-synthase in the transsulfuration pathway - together reducing plasma homocysteine by 20–30% in B-vitamin-replete CKD patients.
